Final Transcripts - DUE August 5, 2009

You must submit an official final transcript of grades to complete your pre-enrollment folder. If you will be awarded a degree, please ensure that the final transcript that you submit indicates this degree.

Please have the official transcript sent directly from your college / university to the address below upon completion of all coursework, and prior to the start of your DDS program.
