Articulator Order - DUE July 24, 2009

Dear Students,

We have arranged a special purchase price for the articulator that you are required to have for the start of fall semester through Whip-Mix Corporation-West. The special school price for the articulator and carrying case is $504.93 as opposed to the retail price of $777.20.  Your total cost with shipping and tax is $557.37.

Please download and print the order form. Complete the "Bill To" information and "Student Name" on the order form. Please include your Master Card, VISA or American Express number with the expiration date, 3 digit security number on the back of the card, and the name of the account with the address and phone number.  Your order will be shipped to the School of Dental Medicine and will be given to you during supply distribution.  Your completed order form must be received no later than July 24, 2009.
